By filling out this form, I agree not to hold Ant’s Happy Endings, its dogs, volunteers, board members, or staff responsible for any accidents, injuries or damages that may occur while volunteering.
If I am volunteering with a child under 18, I understand that I am responsible for supervising my child at all times and agree not to hold Ant’s Happy Endings, its dogs, volunteers, board members, or staff responsible for any accidents, injuries or damages involving my child.
